A 73-year-old man with a history of chronic obstructive pulmonary disease (COPD) treated with chronic antibiotic therapy presents with fever, abdominal pain, and severe diarrhea. He is diagnosed with Clostridium difficile and starts fluids and vancomycin. The next night he is noted to have an irregular pulse with long pauses, and a 12-lead ECG is obtained.
